How many spaces will depend on the form (there is no universal standard), but usually you’ll have enough, and depending on the form you might be allowed to go beyond the official spaces if needed (I remember that once I added one or two extra squares on a form when I ran out of spaces to write; but ask first just in case is not allowed.)
As for the hyphen; you can add the hyphen if you want but the space is accepted just the same.
